    SEND support in schools

    A school SEND team typical consists of these members:

    The graduated approach: assess – plan – do – review

    SEND support follows a cyclical process:

    1. assess needs
    2. plan targeted support
    3. do deliver provision
    4. review impact

    Provision becomes more targeted as understanding develops. Details appear in the school’s SEND policy and information report.

    The SEND Register

    This record lists all learners requiring additional SEND support.

    Pupils are coded:

    • E – EHCP
    • K – SEND support

    The SENCO monitors and updates the register and ensures all staff understand each pupil’s needs.

    K code (SEND support)

    A K code identifies pupils with additional needs who require support through the graduated approach. They usually have an Individual Education Plan (IEP) or equivalent, reviewed regularly with families and external professionals.

    EHCP

    An education, health and care plan is a statutory document for pupils with more specialist needs requiring provision beyond what is available through high quality teaching and school based interventions. The SENCO reviews progress and leads an annual review.
