    Preparing for Adulthood team

    The PfA team supports transition from Children’s Services to Adult Social Care by:

    • working in a strength-based approach to complete a care act assessment which focuses on promoting independence, life skills and wellbeing
    • supporting decision-making and aspirations
    • advising on housing, employment, volunteering and community participation
    • helping to put in place day services and daily living opportunity to enhance wellbeing
    • offering support with commissioning services and direct payment
    • working holistically and providing carers assessment when needed

    Referral and assessment

    The referrals process typically begins at age 16 years, during which:

    • a PfA duty may attend reviews during Years 10 to 12
    • eligibility is assessed under the Care Act 2014
    • up to date consent and financial declaration forms will need to be completed

    Education providers and children's services play a key role in identifying need and supporting referrals.
